The form works great, although with the new WordPress update, the backend of the form no longer works.
Because the developer has an all new purchase system, clients who purchased the plugin in 2013 need to purchase the new plugin to have the ability to place a tech support ticket due to their Order Number no longer being valid.
I placed a message on the free Forum, no response. I emailed the developer twice, no response. I tweeted for help and I was advised to purchase the plugin again to talk to technical support. I asked why I need to purchase, again, the "One time fee". He stated, "One time fee means we don't charge a yearly renewal like every other plugin developer out there."
After research, I do believe I need to update the plugin manually, since it does not allow me to update automatically. It's something my website guy can do, but nonetheless, it would be nice if there was a way I could have some kind of access to tech support, since I am a paying customer.
We use the paid version for a long time and it works fine but with the latest update, some features does not work such as rules, page breaks etc

I don't find a support thread that can solve the emails issue. Am I missing something?
All other features seem to work well.

The plug in's functionality deserves a 5 star rating, but the lack of support drags it down.
The Pro version of this plugin has a lot of potential. I was very excited to try it out for a few different uses on a site - contact us, post submission, managing members, and event rsvps.
This plugin works great for straight forward things like a contact us form that is sent to one email address - whereas I wanted a contact form that allowed the user to choose a topic and have the form sent to an email address consistent with the topic.
Post submission (add-on) worked well.
When creating a series of template RSVP forms I encountered a problem with the duplicate function where the conditional logic rules were not copied.
Being able to view the RSVPs is important for us so I tried the Display Entries add-on and found it too basic to be useful for our purposes. There is not enough control over how the entries are displayed and it doesn't handle fields with the same name (such as 'name' for Person 1 and Person 2).
All of the queries and tickets I submitted about the issues I encountered were responded to in a less than optimal way. Generally, it seems this is a one person show with little customer service skill and low desire to improve things even though the pro plugin is not free and has no trial period.
In summary, it's great for simple, straightforward uses.
There is no option for building a form without spam protection — rather limiting.
Besides that, looks like a pretty good plugin.
Your mileage may vary. In our situation, this very well thought out Plugin had inconsistent issues with:
- the NONCE checking routine
- the server email confirmations routines (not always sending confirmations)
- the Entries index Page in the Dashboard is not well thought out (it simply lists the same confirmation contacts, and not the User specific Form information that would allow sorting.
The issues may be related to our specific mail server (typical LAMP) set-up or Theme conflict, but the Plugin needs to mature more to work with those typical set-ups to be of use to us.
[We did pay the Pro fees, Support is minimalistic. We wanted to love this Plugin, it is sooo close, but not in our specific setup.]
