Came back looking for a new version (as updating site). The developer hasn't updated it in some time, and the links to his site have disappeared (just 404s now).
A pity ... this was a good plugin.
I understand it still works if using shortcodes.
This nifty plugin was just what I was looking for. I'd give it 5 stars if it had better instructions/documentation. Once you get it working, it is very straightforward - just have to follow examples on author's demo website.
I had a small conflict using it with shortcodes buttons but I ended up coding up the button using the HTML5 element anyway - a typical hyperlink wasn't enough in my case, I wanted a big bright red BOOK NOW button.
Depending on your theme, you probably need to touch up the plugin CSS files a bit to make the form elements a touch neater looking.

Could see button on editor, when pushed it opened up a blank popup.
Didnt worked like last version of wordpress. The button in on the editor shows blank white when clicked. So i figured out a way from http://webdesign.myphpmaster.com/form-lightbox-demo/ for the people that the form didnt work can manually put the form by copy pasting shortcodes, but works nicely.
Worked with the plugin I was already using FS Contact form
Having forms display in a lightbox is a basic function in my mind, and this plugin gives you that ability in a very easy way. The button in the editor makes it super simple and I was even able to rework the code a little to add this functionality into one of my template files so I could have a pop-up "Email a friend" functionality. It works great with Gravity Forms. Excellent plugin all around. Thank you!
Played very well with CF7. Used it inside the template - the shortcode works great. Excellent setting page, all in all high quality free plugin.

I love this plugin, and I've used it quite a bit recently. When installed it for the first time I had trouble figuring out how to make it work though--not a very intuitive/user-friendly implementation, but it does its job. Once I understood what to do for set-up it became an essential part of every website.
