Delete and REALLY force the regenerate thumbnail.
This wiped out all my images and drove up my cpu usage so much in the process that my host (Hostgator) shut down my site. I was left with no images, a bunch of broken links, and a huge mess. It took me 2 hours on the phone with Hostgator and a full day to get everything back to normal.
I'm still kicking myself for not doing a backup right before I did this. I had no idea that the plugin would start wiping everything out immediately upon activation. I figured there would be settings, etc. Took me by surprise.
Use this plugin at your own risk.
Deleted old thumbnails but didn't create new ones. So instead of saving me time, this plugin ended up causing me to lose a lot of time. Not to mention, my client's time, which is worse.
