It may not be picking up your theme, but the form can be customized by using CSS to your liking. If you do not know CSS, you can use a plugin – like CSS Hero – in conjunction to help make this process easier. * For the column grid – this is a reported issue that is affecting some users, and we’ve been on it to resolve. For now, I would recommend not using the columns and instead layout vertically. I enabled no-conflict mode in the settings, clicked on save changes, and then proceeded to my forms. Could it be some other setting, plugin or theme that is interfering?
