Don’t know why it was happening, but I went to the page within wordpress, not the page builder, and clicked “update” on the right and now everything is updated. Hopefully I don’t have to do that every time I need to save it.
I had to switch to YIKES Easy Forms for MailChimp, now it works correctly. Not sure what the issue was, but hopefully no one else has the problem. The site is using the Mission WP theme from theme forest, in case you need to know that.
Going to the URL directly works fine. Its http://globalonc.org/cancer-and-you-patient-education-booklets-for-chemotherapy/

Update existing subscribers? Yes.
Replace interest groups? No.
Hide form after successful sign up? Yes.
