This means that your WordPress Permalinks code is being removed from the HTACCESS file and why all pages except for the home page are not loading. You will need to save your Permalinks settings in WordPress again to regenerate the Permalinks code. You could ask your hosting provider if they can see why the HTACCESS file contents are being removed as our plugin won’t do that.

The Rate Limiting rules count mostly normal page loads, but not static assets, and not requests to admin-ajax.php.
It can count 404 Not Found responses on missing static assets though. For example if the theme or a plugin tries to load a missing image then WordPress will try to serve it and will be registered as a 404, and that will be counted.

Currently, the CAPTCHA is not compatible with the WooCommerce option Allow customers to create an account during checkout. it is only applied on the standalone registration form, though the CAPTCHA icon can appear on checkout if Allow customers to log into an existing account during checkout is enabled, because it is applied to logins.
When you said that users who are failing the CAPTCHA when checking out, could it be that they have an existing account, and they are trying to log in during checkout? The login form on the checkout page does check the CAPTCHA, unless there is a conflict with another plugin or the theme.

Thank you for the update. Our plugin may not be the cause. A common cause is WordPress, your theme and all active plugins collectively wanting to use more server RAM memory for PHP processes than is allowed by the web server and activating Wordfence is adding another plugin that is increasing a normal amount of memory.

This is because anti-spam software on your server is using an open resolver when checking the reputation of our sending IP. Their advice to fix this is here:

If a user is required to set up 2FA by the date set by the grace period and they don’t set it up then when that grace period date is reached the user will not be able to login and will see the error below:
LOGIN BLOCKED: 2FA is required to be active on your account. Please contact the site administrator.
You will then have to set them a new grace period on their account profile page.

That query string URL will generate a blank page and thus will never be added to Google’s database of site URL’s to be crawled.
It is used by the plugin to try and detect human activity instead of a bot for the Tools >> Live Traffic page feed reporting.
Because in the past page content was showing for the URL’s and not a blank page then Google deemed these to be a legitimate site pages and why they are showing up in your Search Console now. This was most likely due to a fault in the theme or a different theme you were using and the incorrect use of the WordPress template_redirect hook.
Eventually Google will mark those URL’s with a Soft 404 response and you will have to wait for Google to remove them from its database.
