 * I’m wondering the same thing. I would like the video to not play in tablet and
   mobile devices, only in desktop. I thought I could just hide the video with CSS
   initially, but the fallback image appears as a background image of the div only
   if the video can’t play, not if you choose for it not to play. Thank you!

 * Check the start and end date in the form settings. If it’s not set to a date 
   that includes the date it is today, it won’t show.

 * I’m having the same problem. It creates a new featured image from a video clip.
   But then it does not actually display the video as it should.
 * The weird thing is, it’s working just fine, showing the video, as it should on
   my local copy. When I push it to the live site, it no longer works. It does the
   auto-generate an image thing and puts that in the video’s place.
 * I can’t see that there would be any plugin conflicts causing it, since the plugins
   are the same as the local site. And the media settings are also exactly the same.
 * Here is an example page. The image at the top should be a video:

 * I just tried installing it hopeful that it would do what I need it to. I too 
   am having this problem. It also appears to just not work at all with this error.
   Nothing shows up on the calendar, it does not show list view as I asked it to
   and the buttons at the top do nothing. I’m assuming it’s a version issue as it
   hasn’t been updated since last year.
