Yes, i migrated my blog from wordpress.com to wordpress.org and I am not sure how this markup was came up. Based on your input the issue is with the old Theme data. I appreciate if you suggest me how to get rid of markup code easy way. Now the page had Contact form and i see the message in the plug in submissions list but did not get the email to me.
