 * I’ve suppressed the plugin then installed the plugin again, the plugin is activated
   
   but the menu item doesn’t appear.

    I think your add-on GradeBook does’nt provide 
   all details I need. I will waiting for this feature.

    YES – I set the duration Type of question : ALL (Fill In Blank, True or 
   False, Multiple choice, simple choise)

 * In admin panel, go to theme/menu panel, there is a new option Login/logout.
    
   Open it and click add to menu, and then save your new menu.
