 * I tried entering a fictitious number (28076) in the admin which I think verifies
   when a user enters the same number in order to cut out unwanted requests? The
   user would then need to have that number in advance?

 * Also, I have a question about Must Select a State error. I am not sure where 
   it picked up Shaker Heights (I am assuming that if from the branch location) 
   but it is the only choice and it doesn’t like the choice.

 * I added a couple of more containers after I contacted you and now they show up,

 * I was able to figure out to remove the space above and reduce some of the space
   below the widget, but I would like to make the bottom the same as the top or 
   remove both.
