 * I was having the same issue as many on this forum but found a resolution that
   someone might find helpful.
 * We had recently transferred our website to WPEngine and after we did the plugin
   seemed to stop working.
 * After playing around with this for some time I noticed that if I uploaded a new
   PDF through the Media Library and DID NOT look at the File URL when I would go
   to replace that PDF and then look at the File URL it would be correct. However,
   if I did look at the File URL before replacing the PDF it would not update with
   the new PDF. That’s how I discovered it was a caching issue. However I was clearing
   browser cache, purging the cache through the WPEngine Dashboard, looking at the
   page in other browsers, using incognito mode, and it still wasn’t showing the
   correct PDF when previously looking at the File URL. I then looked at the URL
   on another device, one that had never been to that URL before and boom, the PDF
   shown was the correct one.
 * I found out that when we updated our DNS records when moving to WPEngine we used
   CloudFlare which has it’s own caching service. I purged the CloudFlare cache 
   and that did it. Multiple levels of caching was the culprit. I disabled the caching
   on CloudFlare so I can hopefully avoid situations like this in the future.
