 * Takayuki, I’m not having issues with that section. Those fields you just pasted
   are supposed to be required. I’m having issues with Section 8 labeled “Payment
   Information”
 * Here is the snippet I shared in the first post of the exact location. These fields
   are NOT set to required, but for some reason the form is requiring them to be
   filled in…

 * I ended up creating a #quiz-container to wrap around the quiz elements and I 
   just applied my custom code there, so it was limited to only the quiz – seems
   to have done the trick!

 * Update: Manually deleting and re-installing the WP-INCLUDES folder did the trick!
   I’m back online, all across the board.

 * note: I have also removed all in-active plugins and disabled all currently active
   plug-ins, which seemed to have no effect either.
 * What is in the wp-includes folder? Is it suggested to manually re-install this
   as well, or is there specific information that should not be overwritten with
   the original files?

 * Unfortunately, my Dashboard is still displaying the same broken interface. I 
   have done the re-install both through the Dashboard, and manually, by deleting
   the WP-ADMIN folder and replacing it with a fresh copy.
 * Any other solutions if these do not work?
 * I have cleared my cache and tried it in browsers that have not cached the site
   and it’s still showing broken : /

 * I was about to perform this replacement of “wp-admin” folder but when I logged
   into the Dashboard it had somehow fixed itself.
 * Should I be concerned about this? Is a manual re-install still necessary for 
   safety? I’m not sure how this could have resolved itself.

 * Hi, everyone. I’m having the exact same problem right now.
 * I had experienced this problem after importing 4,000 posts earlier this morning(
   split into a dozen XML files) … something in WP Importer caused my Desktop to
   do this halfway through, which is really frustrating because it took so long 
   to get all the posts re-imported and I don’t want to do it again if this manual
   TOTAL re-install method mentioned above wipes out everything.
 * **Does anyone know what file in particular causes the Dashboard to break like
   this?**

 * the site itself looks totally fine, it’s just the Dashboard that looks broken
