 * I apologize for the lack of info. To give a bit more information: This is happening
   after the file has been successfully uploaded and I receive the alert email with
   the download link. When I click the download link provided in the alert email,
   that error and code pops up.
 * The files themselves are there…I downloaded one and renamed it to the proper 
   extension and it worked. However, the names are being lost. I found the code 
   in the .php file that seems to be causing it…but I am unsure how to move forward
   with fixing it. Any help would be appreciated.
