Are you hosted with WPEngine by any chance?
This client’s install is on there where you’re going to have issue no matter what. The irony is they’re the ones that recommend your plugin on their website.
I’ll try a few other installs on WPE and see what happens.

Well, once you get a staging environment installed you’ll want to turn debug on and see what errors are going on.
For any customization you’re always going to want to use a child theme. Unfortunately, I’m not aware of the Woodmart theme so I can’t comment.
But try debugging when you start testing in staging.
Hi again, sorry to hear this didn’t work.
There are a number of reasons that it could be though. Cache for one. I see you are in SG and their Optimization plugin is enabled. I’d disable that and also turn off cache on the server and purge the cache as well.
Make sure that you are not applying any cache to cart or checkout at all. You can exclude these pages via SG’s optimization plugin once you turn it back on.
You also should be using a child theme if you haven’t started with it yet.
Woodmart should have one.
Lastly, create a staging environment whenever adding or troubleshooting anything on your site as to not affect your live site. It’s easy to create on SG by going to Site Tools, WordPress and Staging. Give it a name. Then click create. Work on that site for all your customizations.
You cleared your server cache and browser cache? You sure your current version is even one version back? Maybe it’s two?
The version prior to 4.1 was buggy as well. What version did you revert back to?
Note, that the plugin works, but it is hiding GF’s default Conditional Logic. Arw you saying for you the GF native CL shows in the backend?

Appreciate you giving me the 411. I’m not concerned with vulnerability then compatibility. The latter is never the first thing I think of. The former is. That’s what gets so many plugin authors in trouble is allowing backdoors by not updating or keeping things tight.
I guess we’ll have to go by what you say. If we find anything though that we feel is an issue we’ll reach back out.

I think you should look at stocky, sellbrite and there was one other I just can’t remember. Both are really good. The latter integrates with WooCommerce, stocky is Shopify only still I think.
We used to use stocky for our 6 locations and our online store.
