The person managing a site I put together wrote to me today with exactly the same problem.
I found that the Jetpack contact form was the problem. Disabling this Jetpack option made everything work again.
Since Jetpack isn’t going to go away and it is a fundamental part of many sites, the problem may be worth investigating.

After more testing:
– send to group is filtered according to the Mass Email setting in User Settings
– send to users is not filtered.Clearly, this is a bug. The Send to Users screen says:
Recipients: You can select multiple users by pressing the CTRL key. When selecting multiple users, any user who should not receive Mass Email will be filtered from the recipient list.
I’m running a *user* custom meta filter, not a group custom meta filter, and I’m using send to users, not send to groups.
Does the Mass Email setting only apply to group emails? That would contradict what is displayed on the send to users screen.

This morning, out of frustration with the discussion here, I went to GitHub, filed bug report, and received a quick response from Jason. Good customer service!
But what is the average user to do? Someone who would never work out how to get to GitHub, let alone know it is there or how to file a bug report? And then they fall prey to misinformation as in this thread.
I’d like better monitoring of the WordPress.org forum by s2.
One site running twenty ten, the other twenty thirteen. Both themes are up to date, unmodified. Not very exotic. Menus on both sites quit working with the same update of s2. Both working again after reverting to v 150102.
Your explanation of strange coincidental behaviour isn’t credible. A strange interaction that has survived a year of use in one case, several years in the other, numerous WP, plugin and theme updates? A strange interaction that keeps menu items perfectly synced with user level, ccaps and page protection?
And if I look in the support forums more than a year ago, before your time, I see people being advised that this does work. That is the reason I and others used it.
I’d like to hear Jason’s take on this.

chevymanusa is correct. The current version breaks this feature.
Downgrading to v 150102 (2015 JAN 02) restores the feature.

That might help in situations where WordPress mail isn’t working. That’s not the case here. WordPress mail works and sends other messages.
The problem is that s2 isn’t calling WordPress mail to send confirmation messages. Adding a plugin that intercepts wp_mail() and sends it via SMTP rather than php mail won’t solve the problem when there is no call to WordPress mail.

The transaction is a paid modification from subscriber (level0) or from level 1 to level 2. s2 should send a Modification Confirmation email.
And in fact it seems to try, as the log file shows:
7 => ‘Modification Confirmation Email sent to: testuser101@xxxxxx.’However, WP Mail Log shows no call to wp_mail(). It is as if the s2 function or method to send mail quits before calling WordPress. Everything else about the transaction is fine. All the user data is updated, the EOT is set correctly, the payment goes through, the log files seem correct. But no confirmation email.
Likewise, new member signup confirmations have quit working.
I think I explained all that above, however it may not be clear to you.
Also, free signup confirmation emails also have quit working.
I am having similar problems. No email message sent on level change. It worked until recently, then mysteriously quit.
I have been through all the troubleshooting steps. I’ve run s2 server scanner, including sending a test email.
I am running WP 4.1, s2 pro v150102.
The log file shows that s2 thinks it is sending the confirmation message:
7 => ‘Modification Confirmation Email sent to: testuser101@xxxxxx.’,The plugin WP Mail Log shows no call to WP to send a confirmation message. The mail log plugin is logging all other messages sent by WP so it is working properly.
This is becoming an urgent problem.
