 * Are any of you guys using a custom template page on the page that contains the
   form? I was having the same issue with a form not showing up on a page that uses
   a custom template and I changed it to use the default template and everything
   started working. Weirdness.

 * Actually, no. I’ve had to completely remove the theme information from my style
   sheet so that my browser isn’t completely filled with my css code when I’m in
   the theme viewer.
 * The problem is definitely rooted within the theme description, I just haven’t
   been able to figure out why.

 * Thanks for the reply, Jeremy. I’ve checked and double checked my theme description
   and I’m still seeing the markup. Is there any other reason that I might be experiencing
   this problem?
