 * Same issue for me: I can successfully submit sites, I get the message “Your URL
   was sucessfully submitted for editor review!”
 * I also receive the e-mail with all the information entered, but I don’t see the
   sites in the admin section, so there’s no way to approve them.

 * Usually this is inserted in the Footer file (footer.php), just above the closing
   body tag.
 * If you only want to count visits, to your main page, put it in index.php.

 * Maybe you could try [the WP-dTree plugin](http://wordpress.org/extend/plugins/wp-dtree-30/)?
   I haven’t tried it myself, but it looks like something you could use.
 * If that doesn’t help, you might want to look into tagging (instead of categories)?
   There are a few plugins to organize them.
