Thanks, forcing a save seems to have fixed it.
To reproduce: I was using AdInserter on wordpress 4.9 and then migrated data to new wordpress (5.3).
Also as a side note, the UI in the settings for Ad Inserter doesn’t display properly with ad block. (although there is a notice warning of that).
