Could had been a conflict with a plugin, however debugging it would had taken forever so I decided to use something else. Thanks anyway 🙂
I solved the issue by adding the “Auto thickbox plus” plugin, im still not sure what caused the problem.

EDIT:
I take that back, the modal overlay opens but after clicking “save”, the note is not added. 🙁
