 * I am using the form on the bottom right footer. Thank you, Mike! I am just trying
   to get rid of the space at the top of the form. The HTML comment only added extra
   blank space.

 * I’m no pro, but it looks like you need to remove the `<pre>` from the beginning
   of the shortcode and the `</pre>` from the ending in the HTML editor.

 * This worked for me too!
