I too have the same problem. It was working fine on 1/3/2015, then I updated to WordPress 4.1 on 1/4 and File Manager is not even listed in my menu and my Download page is blank. I see where you have some conversion steps to go through to upgrade from 2.6 to 2.7. I didn’t do those steps. If that’s my problem, then I have a suggestion for you: in the future, either totally mechanize the conversion steps or offer a completely new version under a new name and leave the old one alone. I’m too used to just hitting the bulk upgrade button.
So what do I do now?

Looks like you got it working. Me too. Finally found the litle icon to add the download to my page up on top. Can’t copy/paste the imbed code – doesn’t work.
