 * Ok – found out that I’m an idiot and reading to fast causes problems.
 * The setting “IMPLICIT” should be set to “NO” if you want the box to appear. So
   problem (self inflicted) solved.

 * We upgraded to WP 4.7 in the interim to see if that helps but no luck.
 * Also we have Enfold as theme with child theme enabled for all modifications. 
   We edactivated Enfold and activated Twenty Fifteen but still no opt-in.
 * If you PM me I can send you the admin login detail as well.
