 * It looks like it may have something to do with field names… I tried creating 
   a new form using the default fields and it submitted no problems… hmmm.

 * Sorry for the vagueness. It was working for a couple of weeks – then it stopped
   working the day I posted the comment. I believe the theme may have been updated.
   I am looking for a previous version to see if I can go back and test an earlier
   version of the theme.
 * Here is a screenshot of the form settings for the form on the homepage that is
   redirecting to an error page:

 * Here is a screenshot of the form settings for one of the forms that is giving
   me the email error:
