 * Im having this same issue. Ive checked an article on iThemes about fixing 404
   lockouts but we do not have any. I’ve had multiple customers tell us they have
   had the lock out screen pop up for no reason.

 * Figured out the issue. I was looking in the wrong direction – it had nothing 
   to do with W3 Total Cache. Around the same time I installed W3TC I also added
   a reCaptcha to my contact form. Once I removed it the form started to work again.
   So I did a little more searching and found some other people having the same 
   issue with the Avada theme and using reCaptcha on a page other than the default
   Avada ‘contact’ page.
 * To fix the issue I removed my recaptcha keys from the Avada theme settings, and
   then added them to the CF7 Integrations settings. Updated the contact form and
   it worked. Not quite sure why it was causing that issue – but guessing the theme
   just didnt want to play nice with the plugin.
 * Maybe it is a good idea to update the orange border FAQ in my first post to include
   something about the recaptcha also being able to kick back an orange border error
   message?

 * I figured out the issue. For some reason the Recaptcha on the form is the culprit.
   When I turn it off the form works correctly. This is not an issue with w3 Total
   Cache.. it is specific to the recaptcha and CF7. Now to try and figure out why
   it is an issue haha But this is not a W3 Total Cache issue, so you can delete
   this thread if you like. Thanks!
