 * I ran into this as well. For me the issue was a period in the keyword.
 * Take for example the keyword: “St. Peters Church”
 * The Focus Keyword field doesn’t seem to like the period. I haven’t tested this
   with any other characters or punctuation.

 * I’m running into the same issue. Any info on a work-around would be greatly appreciated.
 * I considered using a plugin to manage redirects, but I’d rather not add yet another
   plugin.

   trying to fix my issue I think I found your problem. The column header you have
   declared for your hierarchical custom taxonomy is `csv_ctax_(edition)`. Simply
   remove the parenthesis. The header should look like this `csv_ctax_edition`.

 * Re-saving the Permalinks structure in Settings fixed everything.
