Ok I did follow the instructions and managed to put the fields thank you for your help.
I want to add the address fields on the sidebar if you can check it out at http://ambajiusa.org. when you click on donate you see the fields.
Would the above snippet take care of that.

assign your posts to that category
Using custom menu you can add what ever you want posts pages and category or custom links,
click on Appearance>>>>Menus
you can click on my name for example. You will see how I have created sub menu for each category and posts as sub menu.Forum: Fixing WordPress
