Actually, I used [your-name] <[your-email]> in From field and it worked perfectly for me!

Is there a chance to have your nice contact form plugin working with other themes, but default, as well?
I am using https://wordpress.org/themes/basic/ and also receive syntax error on Reply-To: [your-email] in Additional Headers.
