 * We’ve had the same issue, and ended up checking each site every few days. Salesforce
   stayed logged in for a long time when we used the plugin on only one site, but
   the more sites we added, the more often all of them logged out. It helps to open
   a different browser (IE, Firefox, etc.) from the one you’re logged into as a 
   regular Salesforce user, so the login cookies or wherever don’t conflict. But
   since the plugin in unsupported and our Central IT didn’t want to release the
   Gravity+ plugin (written by a small company with very little information and 
   no reviews), we’ve been forced to switch to Form Assembly, which has its own 
   problems.
 * I would check each site daily, log in if logged out, and use the “push to Salesforce”
   button for any entries that didn’t make it due to the site being logged out.

 * We don’t have this problem, as we make all the displayed fields required. You
   could put in a default value for postal code for international addresses.
 * You can also make your own text fields and dropdowns and not use the Address 
   block.

 * Yes, in the Salesforce tab under Forms, add a feed to the Contact object.
