 * I’m using WP 6.4.3 with Unicom theme and PHP 8.0. We are having issues with a
   gravity form – it keeps asking people to login, even though they are logged in.
   I want to disable the Limit Login Attempts plugin to see if that helps and I 
   noticed that in the dashboard, I have two, yes two Limit Login Attempt icons 
   that suggest I have two instances installed. When I go to installed plugins, 
   Limit login attempts is only shows once. I disabled it and uninstalled it and
   now I still have one icon in the dashboard, but nothing shows in the active or
   installed page. How can remove this?
