 * the ‘from’ address of your form should not be the same as the submitter of the
   form. The ‘from’ should appear to come from your wordpress installation.
 * Setup your form so that one of the required elements is the submitters email 
   address. include the [from-email] shortcode in the body of your message, and 
   bingo. You will have the address of the submitter.
 * Just remember, the submitter isnt sending the form … your wordpress installation
   is.

 * i solved my problem … the version of my plugin was too old.
 * I updated to 4.5.1 and i see all the options that I expect.
