DCO Comment Attachment uses the comment_form_submit_field filter to display the attachment field, but your theme don’t use it.
Try to replace
<input name="submit" type="submit" class="button" id="submit" tabindex="5" value="Submit" />
with
<?php echo apply_filters( 'comment_form_submit_field', '<input name="submit" type="submit" class="button" id="submit" tabindex="5" value="Submit" />' ); ?>

You use JetPack for comments. This plugin replaces the standard comment form with its own and does not provide tools for adding additional fields to it.
You can try to disable the comment module in Jetpack settings, after which DCO Comment Attachment should start working.

DCO Comment Attachment uses the standard WordPress features, so it should work correctly with most themes.
I tried to install the Baskerville 2 theme and the file upload field appeared in the comments section. Therefore, probably this is not a conflict with the theme.
Can you please send a link to the page with the problem?

Yes, it is similar to your issue.
( i cannot find the “comment_form”) in the plugin using the plug-in editor.
This function should be in your theme, not in the plugin.
If you still believe it is an issue with the hosting service (i have hostgator), then I will contact them again and hopefully they can help me this time as I have already raised the issue with them before.
Unfortunately, I do not have experience with this hosting. But this seems like a hosting restriction, and I would start the investigation with it.

This is similar to a hosting limitation. Try contacting your hosting provider.

Unfortunately, this is a paid theme, and I can’t download it and try to figure it out 🙁 .
As far as I can see, the seller provides 1 year of theme support. Please try to contact them. Perhaps they can advise something.
