You use JetPack for comments. This plugin replaces the standard comment form with its own and does not provide tools for adding additional fields to it.
You can try to disable the comment module in Jetpack settings, after which DCO Comment Attachment should start working.

Currently, the DCO Comment Attachment supports only standard WordPress comments. But I found on the wpDiscuz site an addon for uploading media files: wpDiscuz Media Uploader.
Also I added the feature request about it. You can subscribe to it to stay turned.

The video is inserted through the video html tag, which supports video in webm and mp4 format. Try converting the video to one of these formats. mp4 looks more compatible.
I’ve added the issue about it (I plan to display such files like file attachments). You can subscribe to it to stay turned.

You can try looking for the phrase “Give a reaction” in your theme files. For example, in a bundled theme twentytwenty the comment form is located in the comments.php file.

The title of the comment form is controlled by the theme used on the site. Try contacting your theme support.

This function in the future enhancements list (you can subscribe on it to stay tuned).
But at the moment you can use the
dco_ca_disable_attachment_field
filter.
For example, this snippet disable the attachment field for all users except users with the “Author” role:$user = wp_get_current_user(); if ( ! is_user_logged_in() || ! in_array( 'author', (array) $user->roles ) ) { add_filter( 'dco_ca_disable_attachment_field', '__return_true'); }

DCO Comment Attachment adds an attachment field and does not affect other fields in the comment form.
