 * Worked through disabling other plugins, **found an unlisted incompatibility issue
   with Wordfence**… Don’t know yet whether there’s options within Wordfence which
   will prevent this issue or not.
 * I had insured that my current user’s 2FA from Wordfence was disabled before I
   started this, so I had hoped that Wordfence would not interfere with the login
   flow required for Duo… That seems not to be the case.
 * Since Wordfence does more for me than Duo, and whilst it’s doesn’t provide Duo
   Push – MFA, it does offer 2FA (TOTP) – so I’m going to struggle to ditch Wordfence
   for Duo at this point.
 * To be clear: **once Wordfence was disabled Duo Universal Prompt worked beautifully**…
   I just can’t use it, as **I don’t want to have to ditch Wordfence**…

   SO much for figuring this out and posting the solution, I had been blind to our
   email copies not making their way out through MailChimp for all this time and

 * Yep, me too, on that version as well – when you use the ‘Test Settings’ button
   after updating the plugin you get: Mailgun HTTP API Test Failure; status “Region
   has not been selected”
 * Fix for me is to hit the Save Button in the settings page, which applies the 
   USA region by default, then re-test… But I’m having to work through each and 
   every single site we admin now in order to work around a breaking update.
