 * Thank you for your quick response.
 * For some reason the social profiles in the Simple Author Box settings were not
   filled in anymore.
    After filling these back in, everything is back normal. Problem
   resolved…

 * Like I said; I solved it by deleting my contact form and create a new one with
   cheboxes from scratch.

 * Solved the problem: there was a problem wit the code in the form. (I copied it
   from one of my other website)
 * Since I tried to create az checkbox from scratch everything works.
