 * Same is happening for me on my site. I had it installed in two locations and 
   it disappeared on the second install. The initial location was the development
   site, and the second location is the actual live site.
 * How can this issue be resolved? We were supposed to launch today and this has
   thrown a huge wrench in our plans.
