 * Thanks muskat, I logged out of admin to view the site and still no luck, same
   thing if I log in with a different user.

 * Thanks muscat, I found the file and moved it.
 * Strangely though, moving it hasn’t made the form appear. I still have a blank
   page where it should be. The shortcode is in and anonymous submission is set 
   to yes. Is there anything I could have missed?
