 * when preview is enabled the notification is also on the live site. Not just on
   the preview URL.

 * The pie chart that is showing up is a placeholder form another plugin.
 * I have removed it and added the shortcode from the Visualizer Plugin that is 
   not working.

 * I am not using anything like that and I have tested all plugins to see if there
   is a conflict. My theme does let me customize the login page a little – added
   a logo. I have reached out to my theme developer because that may be the issue
   but if you had any ideas what I could look into in the meantime that would be
   great.

 * Once I deactivate the plugin, I can log in with no issues.
