I have had trouble with King since 2.2 and all the repairs have not completely worked. I wrote a comment at the King site so I hope he will see it. It is the sign-up form for the email notification plugin. It calls to mallist/index.php. * The comment form and the submit form share common names so I guess that is the problem. You should configure everything from the admin panel Manage –> Democracy. Put what you want in the poll and then activate it. If you have that bit of code in your template the poll should show up (remember, it has to be active).
