 * UPDATE – Did some more experimenting – the form builder doesn’t link the forms
   properly when using Safari – re-tried in Chrome – 100% success.

 * Thanks for the quick response!
 * I had configured correctly, I just did a bit of messing around and changed my
   ABN field name from “abn” to “abn1” and one of my text fields and the GST registered
   radio buttons field started working – I can’t seem to get any other fields working
   yet though, creating new fields doesn’t seem to help either.
 * Also, my configuration form seems to display slightly different to yours:
