    Your csv file should be like this

    “N/A”,”TVSHACK.NET”,”Government Domain Seizure”
    “N/A”,”TVSHACK.CC”,”Government Domain Seizure”

    Please try with this and ensure to have headers in first line. This will solve your issue.
