"You don't have permission to do that" with custom forms

My site was not set up by me pinoakplace.com I recently added a form for my customers to fill out when signing up for my service. They get a message, “You don’t have permission to do that” and I just get there address is my feedback, not all of the other information I want to collect. I also notice this on the contact us part of the page. I don’t know much about website design, just know how to enter content. Is there some sort of setting that I’m missing… ?
