Wrong page after email is entered

I have the website Fat2fitness.net that I installed your plug in on. I set up the plug to work with constant contact correctly. The plug does pop up and allows viewers to enter their email address, however once they do so, they are directed to a page that states:

Please check your inbox right away for the confirmation email we just sent you from Optin Revolution
Subject: “Response Required: Please confirm your request for information.”
(Please also check your spam or junk mail folder as sometimes messages are mistakenly put there.)You MUST confirm your email address. Click the link in the email sent to you. This quick extra step ensures that no one else is signing you up to recieve information without your knowledge. We take your privacy very seriously.
Once you confirm your address by clicking on the link in the email we just sent, we’ll immediately send a follow up email with the information you requested.
Unfortunately, due to the large size of our mailing list, addresses not confirmed within one hour will be deleted.
