Looks like you’re missing the icon font. It might be that some pages have it cached so it’s still showing up for some.
Try switching themes to a standard WordPress theme and see if that fixes it. If that doesn’t fixit, then please let us know what version of WordPress you’re running and what theme you’re using.
Also did you do any updates right before this started happening?
Thanks, Matt! I actually did try all of the standard themes (2011, 2012, etc), but still get the missing items in my dashboard menus and goofy editors (the public sites load fine, however). I tried reinstalling WordPress (4.1.1), but still nothing. No updates before these happened – they’ve beeen at 4.1.1.
Oh, I did upgrade to php 5.3 this past week, but other sites on same server are ok.
One site has 2013 running and the other has an older theme called lightword. Both showing same missing pieces. Thanks for your help!
Just saw a javascript error when trying to run a plugin . . . can’t recreate for some reason. I replaced the js directory in wp-admin, but nothing, FYI>
Reporting in (in case someone is listening) . . . error received on one of the pages is Details: Uncaught ReferenceError: jQuery is not defined

Saw this in another thread on the same problem:
In Firefox, go to Tools, Options, Content. In Fonts & Colors, go to Advanced. Check "allow pages to choose their own fonts...." That took care of it for me.
Might be a browser issue, so maybe try changing settings, or try another browser or computer to see if you have the same problem.
Thanks, Matt . .. that setting is set . . . this happens in all browsers across many wordpress installs on my server – even fresh installs with no plugins . . . I have other VPS installs that all work fine . . . is there a server setting that could be causing the issue maybe?
The dashicon file is at wp-admin/load-styles.php
If you look at the source of a dashboard page do you see a link for that, and are there any errors loading it?
The link should be something like this:

I’m just blown away and so stumped. I have 3 other servers with the same setup and only this server gives this issue (another for instance is that check boxes don’t work in my dashboard too – if that gives any clues).

I’m self hosted (4 VPS for several years now).
That’s what’s so crazy . . . have been installing for many WP sites (I’d say 30 or more) and all has been great until recently with this one server. Checking on a possible cUrl issue . . . other than that, head scratching going on.
If I click on the blog tab I get, https://mooresites.wordpress.com/
And the IP address shows it is at Automattic in San Francisco on a wordpress.com server. Very weird.
Oh, I’m not referring to my blog, it’s for several clients (yes, I run mine on WP.com).
