    Works great and the conditional display of fields is very handy. Love that it writes to a database. My only additional desire that it doesn’t meet is that it would be nice not to have to be logged into wordpress to see the form entries. (Thinking here of staff who should not be logged into wordpress but need to deal with the form results.)

    But all around 5 stars!
