Works as advertised

This has been so needed for Infusionsoft/WP users. Works perfectly. One tweak I wish the author would make: have the form fields be what gets displayed and have the Infusionsoft fields be in the drop-downs. This is a more logical flow for the user and would allow them to map multiple fields for long surveys into the one infusionsoft field like concatenating many survey responses into the contact notes for example. But anyways, brilliant work- good job.
