 * I hope this is the right forum to post this question in, if not please direct
   me to the best place and I’ll make my query there.
 * I am looking for a wordpress intake form for a therapist that inputs user information
   into a service like FileMaker Pro or into some type of non-wordpress database
   or even store the information in the cloud in an excel spreadsheet.
 * I’d like to be able to organize the user’s contact information by name, email,
   phone number etc.. to reference when necessary.
 * Can anyone advise on any suitable plugins for WordPress that would achieve this?

 * Have you looked at Gravity Forms? This is a paid plugin, but offers a lot of 
   functionality for exporting entries as a csv (Excel-readable) file out of the
   box.
 * You can also use a free version of Contact Form 7 + Contact Form Database, but
   this requires a little more configuration work.
