    PHP short echo tag syntax “<?=$variable?>” requires short_open_tag to be enabled, which it may not be depending on hosting. I strongly suggest changing these to “<?php echo $variable; ?>” until PHP 5.4 is in widespread use.

    Sample fix: change
    <input type="text" name="http_domain[]" value="<?=$http_domain?>" />
    into
    <input type="text" name="http_domain[]" value="<?php echo $http_domain; ?>" />
