 * The database table looks like it isn’t created yet.
 * If you haven’t already, configure the Option-> WP Opt-In options please do so.
 * If that doesn’t solve the problem, deactivate the plugin, then reactivate, and
   then set the Option-> WP Opt-In values accordingly.
 * Here’s the installation instructions from the readme.txt file distributed with
   that plugin:
    1. Download and unzip the plugin 2. Upload `wp-opt-in.php` to the`/
   wp-content/plugins/` directory 3. Activate the plugin through the ‘Plugins’ menu
   in WordPress 4. Place the widget in your sidebar and edit its title 5. Configure
   the plugin in the ‘WP Opt-in’ menu located under the WordPress ‘Options’ menu

 * This bug caused problems for some, and has been fixed in version 0.8.

 * The fix could also be to place the php file at specified location in the installation
   instructions.
