WordPress Admin Bar NOT Displaying when Logged In

    WordPress Multisite Version 4.1

    The WordPress Admin Bar is NOT Displaying when logged in. I have tried normal trouble-shooting, and tried fixes mentioned here.

    Show Toolbar when Viewing site is ON
    ALL Plugins have been disabled
    Switched Theme to Twenty Twelve, Thirteen, etc

    No idea what’s going on, should I look at a certain database table or the WP core?
