    OKy never mind, a lot more is broken, by the looks of things every single javascript cant run, so downgrading back…..

    I disabled post thumbnail editor plugin and it worked.

    I have:

    Disabled my themes, and plugins (didnt work)

    Manually re-installed my update (didnt work)

    Reverted to 4.1.1 (didnt work)

    Going back to 4.1 now, All javascripts on my website is not working, not even woocommerce add to cart.

    this is really not nice, It took my work back by about 2 weeks what i can see now, if my suspicions is correct, maybe as far back as 3 months of development is down the drain……

    mine also get back to normal with the disable of “post thumbnail editor”.

    however I asked this question yesterday in stackexchange as bellow,
    http://wordpress.stackexchange.com/questions/185009/insert-media-while-posting-is-not-working-with-new-wordpress-update

    So if anyone found something useful update there as well.

    I have it solves in another way for me.
    And it works. I have the same problem in several project.
    All pictures above 1500px width not works with the upload. All below was ok.

    I have updated the php.ini file in the root of the server, and the wp-admin folder also (placed 2 times).
    Update the server on php 5.5 and it works again.

    php.ini
    upload_max_filesize = 64M
    post_max_size = 64M
    max_execution_time = 300

    But I have also read, that this is an Problem with 1and1 Servers.
